Business Policies for Body and Breath LLC

1. Appointments & Cancellations:

• Appointments can be made by phone, online, or in person.
• A 24 hour cancellation notice is required for all appointments.  Cancellations made with less than 24 hours notice will be subject to a $20 cancellation fee.
• No-shows will be charged the full price of the scheduled service.
• Body and Breath LLC, reserves the right to cancel or reschedule appointments due to unforeseen circumstances.  In such cases, clients will be notified as soon as possible. 
• Late arrivals may result in a shortened treatment time to accommodate other clients.  The full service fee will still apply.

2. Payment & Fees:

• Payment is due at the time of service.
• We accept cash, credit cards, and Venmo.
• Prices are subject to change without notice.
• Gift certificates are available for purchase and are non-refundable.

3. Client Health & Information:

• Prior to your first massage, you will be required to complete a health history form.  Please inform your therapist of any changes in your health or medications.
• It is your responsibility to inform the therapist of any discomfort or pain experienced during the massage.
• Massage therapy is not a substitute for medical treatment.  If you have any serious health concerns, please consult with your physician before receiving massage therapy.
• Body and Breath LLC maintains a professional and ethical environment.  Any inappropriate or suggestive behavior will result in immediate termination of the session and may be reported to the authorities. ABSOLUTELY NO HAPPY ENDINGS, NURU, YONI OR TANTRIC MASSAGE.

4. Scope of Practice:

• Our therapists are trained and licensed to provide therapeutic massage.  They do not diagnose medical conditions or prescribe treatment.
• Massage therapy is intended for relaxation and stress relief, as well as addressing specific muscular discomfort.

5. Confidentiality:

• Client information, including health history and treatment notes, is kept strictly confidential.  We will not share your information with anyone without your written consent, except as required by law.

6. Gift Certificates:

• Gift certificates are non-refundable and cannot be redeemed for cash.
• Gift certificates must be presented at the time of service.

7.  4-Massage Package Terms and Conditions:

• Purchase:  The 4-massage package must be purchased in a single transaction.
• Pricing: The price of the 4-massage package is $280/60 minute, $420/90 minute & $560/120 minute.  This represents a discounted rate compared to purchasing 4 individual massages.
• Payment: Payment for the 4-massage package is due in full at the time of purchase.
• Package Validity/Expiration: The 4-massage package is valid for 6 months from the date of purchase.  Any unused massages will be forfeited after the expiration date. 
• Transferability: The 4-massage package is Transferable.  If transferred, the recipient must be informed of these terms and conditions. 
• Refunds:  4-massage packages are refundable when requested in writing within 3 days of purchase and before any massages from the package have been used, refer to our refund policy for more details.  
• Scheduling: Appointments for massages within the package must be booked in advance and are subject to availability.  Booking should be done in person, by phone, or online.
• Cancellation Policy: The standard cancellation policy (as outlined in the general Terms and Conditions) applies to all appointments booked using the 4-massage package.  Late cancellations or no-shows will result in the forfeiture of that massage session from the package.
• Package Use: The 4-massage package is for individual use by the purchaser/transferee (if applicable) and cannot be shared among multiple individuals.
• Service Type: The 4-massage package is valid for, 60 minute, 90 minute or 120 minute Therapeutic Massages .  Upgrades to longer or specialized massages may be available for an additional fee.
• Combining Offers: The 4-massage package cannot be combined with other discounts, promotions, or offers, unless specifically stated.
• Package Tracking:  Clients can track their remaining massages: by texting Tayna Adams LMT, or by checking their online account.
• Changes to Terms: Body and Breath LLC reserves the right to modify these terms and conditions for the 4-massage package at any time without prior notice.  Any changes will be communicated to package holders.

8. Liability:

• Body and Breath LLC is not liable for any injuries or damages resulting from the use of our services, except in cases of gross negligence.

9. Changes to Terms and Conditions:

• Body and Breath LLC reserves the right to modify these terms and conditions at any time without notice.  Any changes will be posted on our website and in our office. 

10. Governing Law:

• These terms and conditions shall be governed by and construed in accordance with the laws of the State of Utah.
